A discussion with BETELGEUSE prompted speculation leading to the following theory.
Based on the general personality characteristics of Miyuki (down-to-earth: continuing her work as a scientist despite being Tallest), and factors like the Massive* being commissioned from the Vortians as opposed to them being forced to build it, I assume that she could have been a liberal (or even socialist) leader willing to switch away from imperialism in favor of diplomacy, possibly even cancelling the height hierarchy and implementing democratic elections.
This would have made her very unpopular among the military elites, who could have arranged for the fatal accident to occur, either by willfully ignoring Zim's plans or even helping him arrive at the event with his creation knowing that it'd uncontrollably attack the entire audience.
* It's worth noting that despite the Massive being heavily armed, it could have been intended for a different purpose: Irkens do have enemies, so a means of self-defense would be entirely understandable.
